Return to Sport After Unicompartmental Knee Arthroplasty: An Updated Perspective

 Unicompartmental knee arthroplasty (UKA) is a well-established treatment for isolated knee osteoarthritis, particularly in younger and physically active patients. Compared with total knee arthroplasty (TKA), UKA offers several advantages, including faster recovery, preservation of native knee kinematics, and superior functional outcomes in appropriately selected patients. As expectations around postoperative activity continue to rise, the ability to return to sport (RTS) has become a central concern for both patients and surgeons.

Although previous studies and recent meta-analyses have reported high RTS rates following UKA, the existing literature remains heterogeneous and often lacks detailed analysis of key influencing factors. Variables such as patient age, body mass index, treated compartment (medial vs. lateral), and implant design have not been systematically explored. Moreover, several new studies published in recent years further justify an updated and more detailed evaluation of RTS outcomes.

To address these gaps, we performed an updated systematic review and meta-analysis focusing on RTS after UKA. Using a unified and contemporary search strategy, this work was designed alongside a companion review on TKA, allowing for direct and methodologically consistent comparisons between the two procedures. Our analysis provides a comprehensive overview of RTS rates, examines the factors influencing successful return to physical activity, and details the types of sports resumed after UKA.

This updated evidence aims to support informed clinical decision-making and realistic patient counselling, highlighting UKA as a valuable option for patients seeking to maintain an active lifestyle after knee arthroplasty.
